Avenue Hope (Rapid Rehousing Program) – Must transfer from emergency shelter. 12-months of rental assistance along with case management and life skills workshops.

Must be referred by the VA. Housing with intensive case mangement for homeless, disabled veterans and their families. Veterans receive support services at the VA Medical Center, on-site at their apartment complex, and various locations in the community. Provide permanent housing with case management for homeless, disabled veterans and in another program, for their families. Veterans must have a relationship with the VA and must be chronically homeless and disabled.
